Your Right to a Good Faith Estimate
Under the federal No Surprises Act, you have the right to receive a Good Faith Estimate of expected charges before beginning any healthcare services. This applies if you are uninsured or if you choose to pay for services without using your insurance benefits.
The Annex will provide you with a written Good Faith Estimate at or before the time of scheduling. The estimate will reflect the anticipated cost of your program based on the level of care, program length, and services included in your treatment plan.
You may also request a Good Faith Estimate at any time — including before scheduling — by contacting our financial team.

HOW TO Dispute rights
If you receive a bill that is $400 or more above your Good Faith Estimate, you have the right to dispute the charge. Initiating a dispute will not affect the quality of care you receive.
